How To Fill The Illinois Roofing License Application Form?

The Illinois roofing license application is divided into 7 parts. Each section is self-explanatory, but a lot of people make mistakes filling out this form. And the IDFPR can reject an application based on the smallest of mistakes.

We've explained the form section-by-section to help you. No more confusion, no more oopsies, we'll walk you through each section, starting with part 1.

Part I Applicant Identifying Information

Part 1 of the application form is all about basic identification. We'll walk you through each box to make things simple and accurate.

  • Box 1 requires your full legal name. Print your last name, first name, and middle name clearly.
  • Box 2 asks for your Social Security Number or Individual Taxpayer Identification Number.
  • Box 3 is for your permanent mailing address. Include your street, city, state or country, zip code, and county.
  • Box 4 is where you list any maiden names or other surnames you have used.
  • Box 5 requires your mother's original maiden name.
  • Box 6 asks for your specific place of birth. Write the city and state or country.
  • Box 7 needs your date of birth. Use the month, day, and year format provided.
  • Box 8 requires your current age and gender selection. Check the box for female or male.
  • Box 9 asks for telephone numbers where you can be reached. Provide your work, home, and fax numbers.
  • Box 10 requires a valid email address. This field is mandatory for your application.

Every illinois roofing contractor must designate a qualifying party. This person provides their exact name and permanent mailing address. You must ensure the accuracy of the details of risk a rejection.

According to the 2025 Amendment SB 2503, you must also provide a valid email to get your illinois roofing license updates.

Part II Choosing Your Roofing License Exam

Part 2 of the application form is for choosing your license type. You have to include the date and location of the exam, which you get from Continental Testing Services (CTS).

Please add a check mark in the left most table for the type of license you're applying for. There are only two types of roofing contractor licenses, the Unlimited and Limited. Then why are three boxes?

  • Box 1: The Limited Roofing License is for anyone who wants to work primarily on residential projects (8 units or fewer). 
  • Box 2: The Unlimited Roofing License is for people who want to work on all types industrial, residential, and commercial roofing projects.
  • Box 3: It's for people who already have the Limited License but want to upgrade to the Unlimited Roofing License.

Now, please remember, you can only choose the exam that you're actually applying for. So, keep that in mind and cross reference your CTS correspondence for this section.

Part III Identifying Your Roofing Contractor Business

Part 3 is optional in certain situations, like when you're applying as an individual. You only have to fill this section if it applies to you.

Please check the header to see if this section applies to you. It asks for the individual and roofing company where you intend to practice. If you plan on operating individually, skip this section.

  • In Box 1, you need to write the exact name of the licensed roofing contractor.
  • Box 2 requires the address of the contractor business. You must include the street, city, and zip code right here.
  • Move to Box 3 to enter the official name of the contractor business.
  • Enter the license number of the contractor business in Box 4. The form already provides the starting digits 104 for this specific field.

Part IV Personal History Information

The IDFPR requires a background check for every state roofing contractor. You must disclose any criminal offenses or felony convictions. Part 4 asks you specifically for all the necessary background information.

You must answer Yes or No to each question. Please understand you cannot and should not misrepresent any information. Here are all of the questions:

  • Question 1 asks about any criminal convictions or guilty pleas. Do not include minor traffic charges. You must include details about any drunk driving charges.
  • Question 2 asks if you have a felony conviction. You can still get a roofing license with a felony.
  • Question 3 asks about a Certificate of Relief from Disabilities. If you have one, you must attach a copy.
  • Question 4 addresses diseases or conditions that limit your professional ability. This includes mental, physical, or substance abuse conditions.
  • Question 5 asks if you have ever been denied a professional license. It also asks if your license has been disciplined anywhere.
  • Question 6 asks about your discharge status from military or government jobs. You must disclose any discharge that was not honorable.

You must attach a reason, court record (if applicable), and personal statements whenever you answer a question with "Yes."

Part V Child Support and State Tax Details

Part 5 is about child support and state tax details. Please remember, you must answer these two questions because of the law.

  • Question 1 requires you to certify your compliance with any existing child support orders. You must declare under penalty of perjury if you are more than 30 days delinquent. Making a false statement could subject you to contempt of court.
  • Question 2 addresses your compliance with state tax filing and payment requirements. The department will deny your license if you have failed to file a tax return. It will also be denied if you owe taxes or penalties to the revenue department.

Answering incorrectly could automatically get your application rejected. Please answer truthfully and to the best of your ability.

Part VI Application Category Information

Part 6 requires you to select your military application category. You must check the box that best fits your current status. The available options are:

  • Military: Currently or formerly in any of the armed forces wing.
  • Military Spouse: Married to someone in the military.
  • Not Military: Civilian who has never been in the military.
  • Decline to Answer: If its confidential or gagged.

A military service member includes active duty members of the United States Armed Forces, including:

  • The Coast Guard
  • The National Guard. 

This applies if active duty ended within the past two years.

You must provide proof if you select the Military status option. Accepted proof includes a DD214 or a Letter of Service from your commanding officer. You can also use a Proof of Service document from your electronic personnel portal.

Military spouses must also provide specific proof of their marital status. You can use Military Permanent Change of Station Orders showing your name. Other options include a marriage license with assignment change notification or a certified DD1172.

Part VII Certifying Statement and Application Fee

Part 7 is the final certifying section where you bind yourself legally to the application form. You only have to sign the form and include the date of affirmation.

It is crucial that fill out the application form truthfully. Misrepresenting any information could lead to legal troubles when you send this signed application form to the IDFPR.

You must include the required application fee with your submission.
Include a personal statement if you have past criminal convictions. Attach certified court records for these specific cases.

Attach an explanation if you ever faced professional license denial. Include this if you faced professional discipline anywhere.

Provide proof of your active military status if applicable. You can use your DD214 or a Letter of Service.
Include proof of marital status if applying as military spouse. You can use Permanent Change of Station Orders.

You can renew a roofing license by filling about the renewal form. Roofing licenses expire on the last day of December on odd-numbered years (2025, 2027, etc). Failing to renew a license in time will cause a license dormancy.
